On the morning of December 4, 2024, 50 year old UnitedHealthcare CEO Brian Thompson was shot and killed outside the New York Hilton Midtown in Manhattan as he walked to his company's annual investor conference. The masked gunman fled on foot and then on an electric bicycle, triggering a five day nationwide manhunt that ended with the arrest of 26 year old Luigi Mangione at a McDonald's in Altoona, Pennsylvania.

The case immediately became one of the most polarizing in modern American history, prompting a flood of public commentary about health insurance claim denials, folk hero treatment of the suspect on social media, and broader debates about political violence.

Brian Thompson

Brian Robert Thompson was born July 10, 1974 in Ames, Iowa. He grew up in the small town of Jewell, where his father worked at a grain elevator. He was the valedictorian of his South Hamilton High School class in 1993 and graduated at the top of his class at the University of Iowa in 1997 with a degree in business administration.

After graduation, Thompson spent seven years as a manager at the consulting firm PwC before joining UnitedHealth Group in 2004. He spent the next 20 years rising through the ranks of the company, and was named chief executive officer of UnitedHealthcare in April 2021. By the time of his death, his annual compensation reached roughly $10.2 million.

Thompson lived in Maple Grove, Minnesota with his wife Paulette and their two sons. His family later reported that he had received death threats in the months before the killing.

Luigi Mangione

Luigi Nicholas Mangione was born May 6, 1998 in Towson, Maryland. He grew up in a wealthy family that was prominent in Baltimore real estate. He was the valedictorian of his private all boys high school and graduated from the University of Pennsylvania in 2020. From late 2020 to early 2023, he worked remotely as a data engineer for the car shopping company TrueCar.

According to court records and Mangione's own writings on social media, he had suffered from a chronic lower back condition called spondylolisthesis. In November 2024, his mother filed a missing persons report with the San Francisco Police Department after losing contact with him. He was not a UnitedHealthcare customer.

The Brian Thompson Shooting

Mangione traveled to New York City by bus on November 24, 2024 and checked into the HI New York City hostel on the Upper West Side using a fake New Jersey driver's license. According to federal prosecutors, he spent the next several days performing reconnaissance around the New York Hilton Midtown, where Thompson was scheduled to address UnitedHealthcare's annual investors conference.

In the early morning hours of December 4, 2024, Mangione left the hostel on an electric bicycle and rode south to Midtown. He positioned himself between two parked cars on West 54th Street, just outside one of the hotel's side entrances.

At approximately 6:45 a.m., as Thompson walked from his hotel across the street toward the Hilton, the masked gunman stepped onto the sidewalk and opened fire from behind. Thompson was struck in the back and leg. The gunman then fled on foot into a nearby alley before riding an electric bicycle north through Central Park to the Upper West Side. Thompson was rushed to Mount Sinai West Hospital, where he was pronounced dead.

Three bullet casings recovered at the scene were inscribed with the words "delay," "deny," and "depose," widely interpreted as a reference to common tactics that health insurance companies are accused of using to avoid paying claims.

The Manhunt and Arrest

The New York Police Department launched a manhunt and released images of the suspect taken from the hostel. On the morning of December 9, 2024, an employee at a McDonald's in Altoona recognized Mangione as he sat eating breakfast and called the police. Officers arrived and found him "visibly nervous." He was taken into custody on initial charges of forgery and providing false identification to police.

Inside his backpack, officers allegedly found a 3D printed pistol, a 3D printed suppressor consistent with the murder weapon, a fake New Jersey driver's license, a short handwritten letter criticizing the American healthcare system, and a spiral notebook containing writings about the planning of the attack dating back to mid 2024.

The Charges Against Luigi Mangione

Mangione faces charges in three separate jurisdictions. In Pennsylvania, he was charged with firearm and false identification offenses. In New York state, he was charged with first degree murder in furtherance of terrorism, two counts of second degree murder, and multiple weapons charges. In April 2025, he was federally indicted on four charges, including murder through the use of a firearm and interstate stalking resulting in death. U.S. Attorney General Pam Bondi directed federal prosecutors to seek the death penalty.

The case has since narrowed substantially. In September 2025, a New York state judge dismissed the two terrorism related murder charges. In January 2026, a federal district court judge dismissed the firearms charge and the federal murder charge that carried the possibility of the death penalty. Mangione is currently being held at the Metropolitan Detention Center in Brooklyn as proceedings continue.

The Hotel Today

The New York Hilton Midtown remains in operation at 1335 Avenue of the Americas, occupying the entire block between West 53rd and West 54th Streets on the east side of Sixth Avenue. With more than 1,900 rooms, it is one of the largest hotels in New York City and continues to host major investor conferences and corporate events.

The exact location where Brian Thompson was shot is on the public sidewalk along West 54th Street, just outside one of the hotel's side entrances. There is no permanent memorial at the site. Please be respectful of hotel guests, staff, and other visitors at all times, follow all posted signage, and remember that this location remains an active and busy area of Midtown Manhattan.
